坐骨神経痛の治療には外科的アプローチと非外科的アプローチがあります。
研究によると、外科的治療は短期的な痛み緩和に効果的ですが、長期的には保存的治療と同等の結果となります。
多くの患者は手術なしで6~12ヶ月以内に回復します。
保存的治療は時間の経過とともに徐々に、しかし持続的な改善を示します。ほとんどの患者は外科的介入なしに6~12ヶ月以内に顕著な改善を経験します。
早期理学療法(EPT)群は通常ケアと比較して障害スコアの改善が大きく(6ヶ月時点での相対差-5.4ポイント)、治療成功率も高い(45.2%対27.6%)。
硬膜外ステロイド注射については、文献において様々な結果が出ています。有効性を実証した研究もある一方で、統計的有意性を示さず、潜在的な副作用を示唆する研究も数多くあります。エビデンスからは、6ヶ月以上坐骨神経痛を患う患者にはこの注射が推奨される可能性が示唆されています。
外科的治療は症状の緩和が早く、脚の痛みや障害に対して中程度の効果を示します。保存的治療は初期の改善が緩やかです。
外科的治療の効果は持続しますが、保存的治療も着実な改善を示し始めます。両者の差は徐々に縮まります。
長期的な転帰は外科的介入と保存的治療で同等になります。保存的治療は長期間にわたって着実かつ持続的な改善を特徴とします。
初期治療としては保存的治療の方が費用対効果が高いと考えられ、手術は非外科的治療に反応しない患者に限って行われます。研究では、早期の理学療法導入は医療費の負担を軽減し、患者の転帰を改善する可能性があることが示唆されています。段階的治療アプローチ、つまり侵襲性の低い治療法から順に行うことは、坐骨神経痛の患者の多くにとって依然として標準的な推奨治療となっています。
